Clinical practice guidelines for the management of biliary tract and ampullary carcinomas

چکیده
Offprint requests to: T. Takada Although the prognosis of biliary carcinoma remains poor, no clinical practice guidelines for biliary tract and ampullary carcinomas have been published to date. We hope that the publication of Evidence-Based clinical practical guidelines for the management of biliary tract and ampullary carcinomas will contribute to the development of evidence-based diagnosis and treatment in this fi eld. I will describe how the present guidelines came to be published. In 2003, the publication of guidelines was proposed to the Japanese Association of Biliary Surgery as part of an Integrated Research Project for Assessing Medical Technology, sponsored by the Japanese Ministry of Health, Labour, and Welfare. Immediately, a working group was organized for the purpose of preparing evidence-based guidelines. In 2005, the Japanese Association of Biliary Surgery and the Japanese Society of Hepato-Biliary-Pancreatic Surgery (JSHBPS) united, and the preparation of the guidelines was continued by the JSHBPS as its main task. The guidelines were disclosed at the draft stage to facilitate external assessment, and the text: EvidenceBased clinical practice guidelines for the management of biliary tract and ampullary carcinomas was later published in collaboration with the Japan Society of Clinical Oncology and the Integrated Research Project for Assessing Medical Technology, sponsored by the Japanese Ministry of Health, Labour, and Welfare. We believe these guidelines will be of great use and so we decided to publish an English-language version. It is anticipated that, in future, the volume of important evidence and evidence-based evaluations will increase the scope of the guidelines. To allow for the incorporation of evidence as it arises, the guidelines consist of independent articles on important subjects. An abstract of each article is followed by clinical questions to ensure that the guidelines may be used in everyday medical practice.

Purpose, use, and preparation of clinical practice guidelines for the management of biliary tract and ampullary carcinomas
Apart from periampullary carcinoma, the prognosis of biliary tract carcinomas, including hilar cholangiocarcinoma, extrahepatic biliary tract carcinoma, and gallbladder carcinoma, remains poor. Sophisticated diagnostic skills and treatment methods and their application are naturally required to achieve better treatment results for biliary tract carcinomas. However, it is not too much to say tha...
متن کاملAlterations of p16 and prognosis in biliary tract cancers from a population-based study in China.
PURPOSE Biliary tract cancer is an uncommon malignancy with a poor survival rate. We evaluated p16 gene alteration as a prognostic marker for this disease. EXPERIMENTAL DESIGN We studied p16 gene alterations by sequencing, methylation, and loss of heterozygosity of chromosome 9p in 118 biliary tract carcinomas, including 68 gallbladder cancers, 33 extrahepatic bile duct cancers, and 17 ampull...
متن کاملGene Expression Profiling of Ampullary Carcinomas Classifies Ampullary Carcinomas into Biliary-Like and Intestinal-Like Subtypes That Are Prognostic of Outcome
BACKGROUND Adenocarcinomas of the ampulla of Vater are classified as biliary cancers, though the exact epithelium of origin for these cancers is not known. We sought to molecularly classify ampullary adenocarcinomas in comparison to known adenocarcinomas of the pancreas, bile duct, and duodenum by gene expression analysis. METHODS We analyzed 32 fresh-frozen resected, untreated periampullary ...
متن کاملDiagnosis and treatment of choledochoceles.
Choledochoceles are cystic dilatations of the intraduodenal portion of the common bile duct. Although often classified as Type III biliary cysts, choledochoceles have distinctive demographic and anatomic features and a lower risk of malignancy than other types of choledochal cysts. Type A choledochoceles are cystic dilatations of a segment of the intra-ampullary bile duct and are located proxim...
متن کاملGuidelines for chemotherapy of biliary tract and ampullary carcinomas
Few randomized controlled trials (RCTs) with large numbers of patients have been conducted to date in patients with biliary tract cancer, and standard chemotherapy has not been established yet. In this article we review previous studies and clinical trials regarding chemotherapy for unresectable biliary tract cancer, and we present guidelines for the appropriate use of chemotherapy in patients ...
